The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit.
Please arrive for your appointment no more than 5-10 minutes early. If you are a new patient or have information that has changed (ie: new insurance, new address, etc) try to come 10-15 minutes early in order to fill out necessary paperwork.
Please have your current health insurance card with you at the time of your exam.
We ask for your cooperation with annual exams. At the time of your annual, we will have you self-address a post card appointment “reminder” and a patient information sheet. (You will receive your reminder card 3 months before your annual exam is due.) Please call to schedule your next annual exam as soon as your receive your reminder card.
At the present time, there is a shortage of female gynecologists. We have a staff of one physician, one physician assistant, and a nurse practitioner. Our health care providers are all board certified, exceptionally well trained, and professional. Therefore, we alternate annual exams between the Doctors and one of our health care practitioners.
Problem visits can be scheduled with one of our providers due to our policy of scheduling annual visits 3 months in advance.
Cancellations less than 48 hours before a visit will incur a $30 cancellation fee. However, there will be no cancellation fee for inclement weather, illness of self, or death/illness of an immediate family member.
We normally ask for your address, phone number, date of birth, and insurance card information to process your bill with your insurance company. A social security number is requested to process your pap smear or specimen with the lab and/or to schedule surgeries. This information is kept private and protected and is not shared with any other business. Our office operates under the guidelines set forth by HIPAA.
